Questions about the numbers.
How is the RPM, RTM, and CCM revenue estimate calculated?
The estimate multiplies your patient counts by the 2026 CMS Medicare Physician Fee Schedule national average allowable for each applicable CPT code, then subtracts the published Neuvora fee schedule. It assumes each enrolled patient meets the minimum monitoring days and clinical time thresholds required to bill that code. Actual reimbursement varies by geographic locality, payer mix, and how many patients meet those thresholds in a given month.
Which CPT codes does the calculator use?
RPM uses 99453 for setup, 99454 for device supply, and 99457 and 99458 for treatment management time. RTM uses 98975 for setup, 98977 for device supply, and 98980 and 98981 for management time. CCM uses 99490 and 99439. The 2026 short-duration codes (99445, 99470, 98984, 98979) are included in the coding reference tab.
Can the same patient be billed for RPM and RTM in the same month?
No. RPM and RTM cannot be billed for the same patient in the same calendar month. The calculator reflects this by treating combined-service patients separately from single-service patients, so a patient is never counted under both.
Do I need a minimum number of monitoring days to bill?
Device supply codes 99454 and 98977 require 16 or more days of readings in a 30-day period. The 2026 codes 99445 and 98984 cover shorter 2 to 15 day periods and cannot be billed in the same period as their 16-day counterparts. Treatment management codes additionally require at least one interactive communication with the patient during the month.
Are these Medicare rates or commercial rates?
The calculator uses 2026 Medicare Physician Fee Schedule national averages. Commercial payer rates differ and are negotiated per contract. Always verify current rates with your local Medicare Administrative Contractor before relying on a projection.
What does Neuvora charge, and is it included in the estimate?
Yes. The net figure already has Neuvora fees deducted. The fee schedule is published in full on the Neuvora fees tab of this page, covering the per-patient admin fee, monitoring support rate, and monthly maximums for combined and single-service patients.
