Comprehensive care plan
A comprehensive care plan is the documented, patient-specific plan covering the conditions being managed, the goals of treatment, and the interventions — established, revised, and made available to the care team.
Why it matters
It is a billing requirement for chronic care management, not just good practice. A CCM claim without a care plan that was genuinely established and revised is a claim at risk.
