A CPT code is the standardised five-character identifier used to report a medical service to a payer. Each code carries its own requirements for time, documentation, and who may furnish it.
Why it matters
For monitoring programmes the code is rarely the hard part — the requirements attached to it are. Most denied claims fail on documented time or consent, not on code selection.
